There are more upscale hand-rolled cigars like CAO and Acid that are infused with dessert flavors, but the smoke still smells "cigarry". It all comes down to the types of tobaccos used, and the aging, curing, etc. Hand-rolled cigars use whole leaves of ligero, Habano, Sumatra, etc, that give off an earthy funky smell when you light them up. Those classic "Grandpa" sweet pipe blends are usually made from shredded Burley & Virginia tobacco leaves that get tossed with flavoring liqueurs before being packaged and shipped. And for the record there are some hefty pipe tobaccos out there that smell like someone accidentally lit their baseball glove on fire and tried to douse the flames with A1 steak sauce.
